Some of the ingested inositol hexanicotinate is hydrolyzed in the body to slowly release free niacin and inositol. (1,2) 

However, because much of it remains intact in the body, it's thought that inositol hexanicotinate may exert direct effects in addition to those of its constituent inositol and niacin fractions, without causing the characteristic niacin flush. (2)
